Booking forms

Booking and qualification forms that keep every lead

A typical form loses everything when the visitor stops halfway. Ours save the name and phone number first, then qualify, then book. The same line of code puts them on any website.

Why the phone number comes first

Every step of a form loses some visitors. The ones who stop near the end are often the most interested: they read the offer, answered the questions and were choosing a time. If the form only saves at the last click, those people leave no trace.

Our forms save the name and phone number at the first step, before the calendar and before the later questions. A visitor who drops out is still a lead in the CRM, and someone can call while they still remember asking.

That call can finish the booking the form did not. Our lead handling work is built around making it quickly.

What the forms do

Lead saved at step one

Name and phone number are stored before any later question or the calendar, so a half-finished form still reaches your team.

Qualifying questions

Multiple choice, ranges and dates. A disqualifying answer can be handled differently from a qualifying one.

Real free times

The form can show your team’s actual free slots and book straight into the calendar, with a minimum notice time so nobody is booked before your team can prepare.

Source on every lead

Each lead lands in the CRM with the page, the referrer, the UTM parameters and the ad it came from.

Qualified leads back to Meta

Leads that meet the form’s rules are reported to Meta through the Conversions API.

Visit tracking

Each page, click and form step is recorded. A visit ends after 30 minutes without activity, and its source is classified as AI assistant, search, social or ads.

One snippet, any website

The form goes on WordPress, Wix, Next.js, Astro or a plain HTML page with the same one-line snippet. It runs on Napexy’s servers inside a frame, so your site needs no plugin, and changing the form does not mean touching the site.

If the form cannot load, a fallback shows in its place, for example your phone number. The visitor still has a way to reach you.

For campaign traffic, we build landing pages around the form, so the page answers the ad and the booking is one step away.

Teaching Meta which leads matter

Meta’s ad delivery optimises toward the event you report to it. Report every form fill and it looks for more people who fill in forms, including the ones who were never going to buy.

Our forms report the leads that meet the form’s qualification rules back to Meta through the Conversions API. Campaigns can then optimise toward the leads that fit your offer. Choosing which campaigns use that signal is part of our Facebook and Instagram ads work.

How we set up a form with you

  1. Agree the questions

We write the qualifying questions with you: what makes a lead worth a call in your business, and what rules one out.

  2. Set the rules

Which answers qualify, which disqualify, and how each one counts toward the lead score in the CRM.

  3. Connect the calendar

Your team’s working hours, unavailable times and the minimum notice before a booking.

  4. Embed and check

One line on your site, a fallback in place, and a test lead followed into the CRM with its source.

Frequently asked questions

How much do the booking forms cost?

They have published plans: Startup, Growth and Business, priced per seat per month, with 20% off when billed yearly. Each plan includes the CRM and the inbox. See booking form pricing.

Do I need a plugin to add the form to WordPress?

No. The same one-line snippet works on WordPress, Wix, Next.js, Astro and plain HTML. The form runs on our servers in a frame, so there is nothing to install on your site.

What happens if a visitor stops before booking?

They are still a lead. The name and phone number are saved at the first step, so your team can call and finish the booking by phone.

Can the form book appointments directly?

Yes. It can show your team’s real free times and book into the calendar, with a minimum notice time between the booking and the appointment.

What if the form fails to load?

A fallback shows in its place, for example your phone number, so the visitor can still reach you.

Which leads are sent to Meta?

The ones that meet the form’s qualification rules. They are reported through the Conversions API, so Meta’s campaigns can optimise toward the leads that fit your offer. Our conversion rate optimization work then looks at where the rest drop out.
